PharmacologyAmbroxol, the active metabolite of bromhexine, has been demonstrated to have a stronger bronchosecretolytic effect than bromhexine. It enhances sputum rheology by hydrating mucus, resulting in its liquefaction within the respiratory tract, which aids in mucus expulsion and alleviates dyspnea. Additionally, ambroxol stimulates alveolar cells to produce surfactant phospholipids, thereby reducing surface tension in the alveoli. It also diminishes bronchial hyperactivity and possesses anti-inflammatory properties due to its inhibitory effect on the production of cellular cytokines and arachidonic acid metabolites. In patients with COPD, ambroxol traditionally improves airway patency. 
Indications

The conditions include:

  • Productive cough
  • Acute and chronic inflammatory disorders of the upper and lower respiratory tracts involving thick mucus, such as acute and chronic bronchitis
  • Inflammatory diseases of the rhinopharyngeal tract (laryngitis, pharyngitis, sinusitis, and rhinitis) associated with thick mucus
  • Asthmatic bronchitis and bronchial asthma with dense expectoration
  • Bronchiectasis
  • Chronic pneumonia, among others.
 
Dose and Administration

Average Daily Dose (Preferably After Meal):

Pediatric Drops:

  • 0-6 months: 0.5 ml twice daily
  • 6-12 months: 1 ml twice daily
  • 1-2 years: 1.25 ml twice daily

Syrup:

  • 2-5 years: 2.5 ml (1/2 teaspoon) 2-3 times daily
  • 5-10 years: 5 ml (1 teaspoon) 2-3 times daily
  • 10 years and older, including adults: 10 ml (2 teaspoons) 3 times daily

Sustained Release Capsule:

  • Adults and children over 12 years: 1 capsule once daily
 
Contraindications

Should not be used in individuals with known hypersensitivity to Ambroxol or Bromhexine.

 
Warnings & PrecautionsAmbroxol should be administered carefully to patients with gastric or duodenal ulcers or convulsive disorders. Additionally, individuals with liver and kidney impairments should use it with caution. 
Side EffectsOccasionally, gastrointestinal side effects such as epigastric pain and a sensation of stomach fullness may occur. Rarely, allergic reactions including rash, hives, or angioedema have been reported. 
Use in Pregnancy & LactationStudies on teratogenicity and fetal toxicity have indicated that Ambroxol does not cause harm. Nonetheless, its use is not recommended during pregnancy, particularly in the first trimester. Additionally, its safety during breastfeeding has not been determined. 
Storage ConditionStore in a cool, dry place, away from light and heat. Ensure it is out of children’s reach.
